I give this declaration and agree to these terms in support of my application for enrolment in a course at Macleay College.

I authorise Macleay College to obtain any further information or records from any educational institution or recognised educational qualifications assessment body and to verify any information about my employment history that I have given to Macleay College.

I authorise Macleay College to contact me via phone, SMS or email.

I authorise Macleay College to access the Visa Entitlement Verification Online (VEVO) system to check my visa details.

I agree that I have been given the opportunity to read the policies, procedures and terms set out in the Macleay College Student Handbook available via the Forms & Policies page and agree to abide by them if I am accepted as a student at Macleay College.

I understand that some Macleay College courses require the completion of practical internship experience and that, if this is a requirement of my course, I will be required to complete all declarations, consents and checks required by law or otherwise reasonably required by Macleay College.

I acknowledge that I have read and accepted the terms of Macleay College’s Privacy Policy published on the College’s website.

I acknowledge that Macleay College reserves the right at any stage to vary or reverse any decision regarding admission or enrolment made on the basis of incorrect, incomplete or fraudulent information.

I acknowledge that the College reserves the right to alter any course, subject, admission requirement or fee without notice.

I declare that I am a Genuine Temporary Entrant and Genuine Student and that I have read and understood conditions relating to these requirements on the Department of Home Affairs website: www.homeaffairs.gov.au

I understand that the College may choose not to issue an electronic Confirmation of Enrolment (eCoE) if the College, its agent or its nominee assesses that I am unlikely to meet the Genuine Temporary Entrant requirements for a student visa, as required by Department of Home Affairs.

I understand and accept that I must abide by all terms and conditions of my visa.

I am aware of the tuition, travel and living costs for my stay in Australia and that it is my responsibility to meet such costs for the duration of my program.

I agree to advise the College within seven days of any subsequent changes to my residential address in Australia.

I acknowledge that the information I provide to the College may be made available to Australian Government agencies, pursuant to obligations under the Education Services for Overseas Students Act 2000 and the National Code of Practice for Providers of Education and Training to Overseas Students 2018 recognise that disclosure to Government agencies can include, but is not limited to, information regarding breaches of a student visa condition, changes to my enrolment and/or for visa processing purposes.

I understand that any documentation I submit becomes property of Macleay College and will not be returned to me.

I acknowledge that due to privacy regulations, the College cannot disclose information about me to any third party such as parents, friends or relatives without my written consent.

I understand that this declaration does not remove my rights as a student to take action under Australia’s consumer protection laws, including any statutory cooling-off period if one applies.

I declare that the information I have supplied on this form is complete, true and correct.
